What information is available for patients?

There is lots of info available about the NHS Care Records Service.

All patients should have received a ‘patient pack’ through the post, although we do have to appreciate that some may go astray or be delivered to an old address if you haven’t advised your GP of a move etc. So there are PDFs of all the documents available here:
