HealthSpace in Bolton

Advanced HealthSpace Account

In addition to all the great features available on the basic HealthSpace, if your GP has created an NHS Summary Care Record for you then you can register for an Advanced HealthSpace account and view your record.

There is, understandably, more involved in the advanced account registration process. Because the account gives you access to some of your key medical information we need to verify the applicant’s identity before access is granted.

The application process is as follows:

  1. You must first register for a basic HealthSpace account. Go to the web site and follow the onscreen instructions.
  2. Once you’ve registered for a basic account and activated it the next time you log in you’ll be offered the chance to upgrade to an advanced account, if you’re registered with a GP practice in the Bolton area.
  3. Follow the on-screen instructions and fill out the application form. You’ll need to print off the completed application form or, if you don’t have access to a printer, make a note of your application number.
  4. You’ll need to get three forms of ID together to proceed with your application - click here (link to Accepted ID page) to find out which forms of ID we accept – it’s important that you check because we can’t process your application form without the right ID.
  5. You then need to take your application form, or a note of the number, along with your three forms of identification to Bolton’s HealthSpace Registration Office.
    The Registration Office is based in the PCT’s public health library, which is in Bolton Central Library, Le Mans Crescent, in the town centre.
  6. A HealthSpace agent will check your application form and verify your identity using your ID documents. Once this is done the agent will send your forms to the HealthSpace processing office and hand you a leaflet containing more information along with a secure log-in card.
    You’ll need to keep the card safe as you’ll need it to complete the registration process, and also every time you log in to HealthSpace to view your Summary Care Record.
  7. The HealthSpace processing office then makes final checks of your form and creates your account. Once this is done you will be sent a confirmation letter and activation code through the post, usually within 2-4 weeks. The letter will explain how to complete your registration.

If you don’t receive your activation through the post four weeks after visiting the Registration Office, please let us know by:

  1. Visiting www.healthspace.nhs.uk and clicking on the ‘Feedback’ link at the top right of the screen. Be sure to include your full name, address, date of birth and details of where you visited the HealthSpace agent (i.e. Bolton Central Library); or
  2. Calling the HealthSpace customer support line on 08453 665665.
