What ID do I need for Advanced Account registration?

You need to bring three different proofs of identity with you when you visit the HealthSpace registration agent at the Central Library.

The reason we ask for this ID is due to the fact that we’re dealing with patient’s key medical information so we must be sure that you are who you say you are and that you live where you say you live – for both your peace of mind and ours!

You need to bring one item from this first list:

Proof of Identity (this shows us that you are who you say you are)

  1. current signed passport;
  2. residence permit issued by Home Office to EU Nationals on sight of own country passport;
  3. current UK photocard driving licence;
  4. current full UK driving licence (old version) – old style provisional driving licences are not acceptable;
  5. A recent bank, building society or credit union statement (within last three months) or passbook containing current address;
  6. recent Inland Revenue tax notification;
  7. birth certificate;
  8. HM Forces Identity Card.

You also need to bring two items from this second list:

Proof that you are active in the community (this shows us that you live where you say you live)

  1. recent (within 3 months of the current date) original utility bill or certificate from a utility company confirming the arrangement to pay for the services at a fixed address on prepayment terms (note that mobile telephone bills should not be accepted as they can be sent to different addresses and bills printed from the internet should not be accepted as their integrity cannot be guaranteed);
  2. local authority tax bill (valid for current year);
  3. current UK photo card driving licence (if not used for evidence of name);
  4. current full UK driving licence (old version) (if not used for evidence of name);
  5. bank, building society or credit union statement or passbook containing current address;
  6. recent (within 3 months of the current date) original mortgage statement from a recognised lender;
  7. current local council rent card or tenancy agreement;
  8. current benefit book or card or original notification letter from the Department for Work & Pensions confirming the rights to benefit;

Change of Name: If your current name is different to the one shown on any of these documents, you must show proof of your name change. Examples of this are: an original marriage certificate, decree nisi or absolute, or deed-poll declaration.  You must be able to show clear link between the name on your proof of personal identity and your current name.

Young people between the ages of 16 and 18 can also use the following documents, providing they are dated in the last 6 months:

  • Educational qualifications - original exam certificates issued by a UK examination board;
  • A formal letter from a human resources department confirming an offer of employment. The letter must be on headed notepaper and include contact details.

To reiterate – you must bring one item from list A AND two items from list B, so that’s three different pieces of ID in total.

Some items appear in both list but can only be used once – for example if you bring your photocard drivers license as your list A ID, you cannot show it as one of your list B IDs too, you will need two different forms of ID for your list B items.

We ask for these documents to protect you and your identity. We can’t process your application unless you have the right forms of ID with you.
