Diabetes care

Care for people with diabetes in Bolton is provided through joint working between different professional groups and different health organisations, in partnership with the person with diabetes. All the staff involved work together in an integrated way to ensure the best care for people with diabetes locally. Different people with diabetes require services that respond to their specific needs, this varies from person to person and therefore not all people with diabetes will receive exactly the same services. Our local team approach is aimed at promoting good health and preventing complications for people of all ages with diabetes. The emphasis on diabetes education encourages self management and control of diabetes using information and practical help for people with diabetes and their carers. We are working to improve services locally and ensure that our diabetes services will meet the standards set out in the National Service Framework for Diabetes.
