Diabetes Services

Your Full Diabetes Team:

The person with diabetes who is essential to ensuring that their diabetes is as well controlled as possible through living a healthy lifestyle and understanding how their diabetes affects them individually.

From your GP surgery (normally you see these people either at the surgery or at your home)

  • Practice nurses
  • GPs

From the specialist team (these staff are based at the diabetes centre but are very flexible and may work in your GP practice, at the diabetes centre or in the hospital)

  • Nurses
  • Podiatrists (foot experts)
  • Dietitians (food and nutrition experts)
  • Doctors

From the general community team (these staff mostly work in health centres or may visit you in your own home if you are housebound)

  • District nurses
  • Podiatrists (foot experts)
  • Dietitians

From the hospital (these staff work mostly at the hospital)

  • Ward staff throughout the hospital
  • Specialist staff in cardiology (for heart and vascular related complications)
  • Specialist staff in nephrology (for renal/kidney related complications)
  • Specialist staff in ophthalmology (for eye related complications)
  • Specialist staff in obstetrics (for pregnancy related issues)

From the eye screening team (these staff are based at the hospital and work there and throughout Bolton in health centres)

  • Photographers
  • Nursing staff
  • Doctors in ophthalmology

In the background these staff are supported by administrative, clerical and management staff and a team who co-ordinate the diabetes information system to allow this integrated service to work.
