Diabetes ServicesThe Specialist Diabetes ServiceThe Diabetes Specialist Team provides specialist and expert services to patients with diabetes. These services are provided directly to patients through daily clinics at the Diabetes Centre on Chorley Street near the town centre. Services are also provided through staff visiting the hospital to see inpatients with diabetes, particularly those with complex needs. This community based team, works closely with family doctors and other primary care staff and with hospital based colleagues. This allows an integrated service to be offered to patients, ensuring that any individual with Diabetes who lives in Bolton will receive a high standard of holistic care irrespective of where they receive it. We aim to work together to benefit patients and continue to improve the service we provide. As an equally high priority, the team provides specialist and expert advice and education to other healthcare professionals across the Bolton health and social care community regarding diabetes related issues. The team participate in a range of research trials linked to the service area. Management of Bolton's Diabetes Centre transferred on 1 April 2004 from the Bolton Hospital's NHS Trust to Bolton Primary Care Trust. The Specialist Diabetes TeamDoctors
