Long Term Conditions (Neurology) Service
Who is the service for?
We provide a medical nursing and therapy service for patients with a
diagnosed neurological condition. Who are resident in Bolton and/or
registered with a Bolton GP.
Aims of the Service
- To provide assessment, treatment and on going review/care
for patients.
- To link with Regional Services to ensure appropriate care.
