Social Prescribing Project

Bolton’s Social Prescribing project has been developed in response to the Health and Well-Being Consultation undertaken as part of the regeneration strategy developed by East Bolton Regeneration. The consultation highlighted the links between social and emotional problems on an individual’s health and well-being. Local community members acknowledged worries around debt, social exclusion, child rearing, community safety to mention a few, all of which have a negative impact on an individual’s health and well-being.

The Health Topic group worked with local staff and the community to develop a particular model of Social Prescribing that provides a formal framework enabling primary care workers to refer clients to non-clinical services, addressing some of the socio-economic and emotional problems.

Bolton’s model of Social Prescribing includes the use of a Directory to help health care professionals identify an appropriate project/service for their client and make a referral.

Clients have the option of self referral to most of the projects.
