Referral Process

• The person making the referral will identify a suitable project for their client using the Social Prescribing Directory
• The referrer will then complete a referral form as directed in the Social Prescribing Directory. Please ensure you complete all sections including patient’s signature to consent to sharing of information with the project
• 1 copy of the referral form will be sent to/kept by:
  • Client
  • Referrer
  • Project
• The client will be contacted by the project as detailed in the Social Prescribing Directory.
• The project will be responsible for the client who is attending their project.
• The project will feed back to the referrer within 4 weeks of the referral being received by project, using the feedback form.
